Roles & Responsibilities

• Conduct thorough speech-language evaluations for skilled nursing facility residents
• Provide daily, individualized treatment sessions to promote patient progress
• Prepare timely, accurate documentation and manage discharge planning
• Collaborate with the multidisciplinary therapy team to optimize outcomes
• Follow CRG policies & procedures and meet the facility productivity requirement (90%)

Qualifications

• 1-3 years of SLP experience (as listed in the job order)
• Prior SNF experience strongly preferred
• Licensed Speech-Language Pathologist (state licensure/credentials as required)
• Comfortable following facility dress code (black scrubs) and being onsite to receive hours
• Strong documentation, communication, and teamwork skills

Knowledge & Skills

• Skilled SLP evaluations and treatment planning
• Daily treatment delivery and discharge preparation
• Experience working in a skilled nursing facility (SNF) environment
• Familiarity with productivity expectations and clinical documentation standards
• Ability to follow company policies (CRG P&P) and work within a team-based therapy model
