Roles & Responsibilities

• Provide high-quality physical therapy treatment under the supervision of a registered PT according to the established plan of care
• Implement therapeutic exercises, gait training, transfers and ADL training to restore function and reduce pain
• Deliver therapeutic modalities as prescribed, including use of approved equipment like ultrasound and low-voltage devices
• Perform clinical assessments, observe patient response, and report changes to the supervising RPT
• Teach patients and caregivers self-care techniques and home exercise programs to support recovery
• Complete accurate, timely clinical documentation in accordance with agency standards
• Coordinate equipment needs with the health agency to support patient independence
• Collaborate with interdisciplinary team members to ensure compassionate, person-centered home care

Qualifications

• Unencumbered PTA license in Vermont and New Hampshire or a compact license (required)
• Graduate of an accredited Physical Therapy Assistant program (required)
• Home health experience as a PTA preferred (around 1 year preferred)
• Demonstrated ability to perform therapeutic exercises, gait training, and prosthetic training
• Strong clinical documentation skills and ability to communicate effectively with patients, caregivers, and the supervising PT

Education

• Graduate of an accredited Physical Therapy Assistant program (associate degree or diploma)

Knowledge & Skills

• Skilled in therapeutic exercises, gait training, transfers, and ADL training
• Experience with therapeutic modalities and rehabilitation equipment (e.g., ultrasound, electrotherapy)
• Proficient clinical documentation and timely charting
• Ability to assess patient response to treatment and escalate changes appropriately
• Patient and caregiver education techniques and pain management strategies
• Comfort working in home health settings and adapting to varied home environments
