Roles & Responsibilities

• Assess and evaluate resident mobility, function, and rehabilitation needs using evidence-based practices
• Develop and implement individualized treatment plans that improve independence and quality of life
• Deliver hands-on therapy, therapeutic exercise, gait training, and assistive device training
• Document resident progress clearly, accurately, and on time in the medical record
• Collaborate with nurses, occupational therapists, speech therapists, and other team members to coordinate care
• Educate residents and families on safe mobility, fall prevention, and home exercise programs
• Advocate for resident-centered care and contribute to a positive, team-focused environment

Qualifications

• Current Physical Therapist license in Montana or eligibility to obtain Montana licensure
• Active Basic Life Support (BLS/CPR) certification or willingness to obtain
• Experience in inpatient, long-term care, or geriatric rehabilitation preferred
• Strong documentation and clinical reasoning skills
• Excellent communication and teamwork abilities

Education

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required

Knowledge & Skills

• Geriatric rehabilitation and restorative care approaches
• Therapeutic exercise prescription and progression
• Manual therapy basics and modalities relevant to long-term care
• Safe transfer and gait training techniques
• Skilled documentation and outcome measurement
• Ability to work collaboratively in an interdisciplinary environment
