Roles & Responsibilities

• Provide compassionate direct patient care including vital signs, medication administration, and wound care
• Collaborate with RNs and interdisciplinary teams to implement patient care plans and ensure continuity
• Document patient status and interventions clearly and promptly in the electronic health record
• Monitor patients for changes in condition and escalate concerns to the appropriate clinical staff
• Support safe patient transfers, admissions, and discharges with attention to detail and dignity
• Educate patients and families on care plans, medications, and follow-up needs in a clear, respectful way

Qualifications

• Active LPN license (practical/vocational nursing) — New Hampshire licensure preferred
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Clinical experience in a long-term care, skilled nursing, or similar setting preferred
• Up-to-date immunizations and ability to meet facility onboarding requirements
• Strong communication skills and a patient-centered attitude

Education

• Practical Nursing diploma or certificate (LPN/LVN program)
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and dressing change techniques
• Ability to take accurate vitals and recognize clinical changes
• Familiarity with electronic health record charting
• Teamwork, time management, and prioritization in a fast-paced setting
