Roles & Responsibilities

• Evaluate patients comprehensively and identify functional limitations and rehabilitation goals
• Develop individualized treatment plans that align with physician orders and state regulations
• Provide skilled physical therapy interventions across inpatient, swing bed, outpatient, and skilled nursing settings
• Document assessments, plans, progress, and discharge summaries accurately in the EMR
• Collaborate with physicians, nurses, therapists, and the care team to optimize patient outcomes
• Educate patients and their families on home programs, safety, and ongoing recovery strategies
• Participate in continuing professional development and quality improvement initiatives

Qualifications

• Active California Physical Therapy License in good standing
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA Basic Life Support (BLS) certification
• 1+ year experience in acute care, swing bed/skilled nursing, or outpatient settings
• Proficiency with EMR systems and basic computer skills
• Strong clinical reasoning, communication, documentation, and problem-solving skills

Education

• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program

Knowledge & Skills

• Patient evaluation and treatment planning
• Clinical reasoning for diverse diagnoses and populations
• Accurate and timely documentation
• EMR proficiency
• Experience across inpatient, outpatient, swing bed, and skilled nursing care
• Attention to detail and effective interdisciplinary communication
