Roles & Responsibilities

• Conduct comprehensive clinical screening, assessment, diagnosis, treatment planning and brief therapy for patients from birth through elder.
• Lead and facilitate discharge planning and care transitions, including ALF and transitional placements.
• Coordinate care with hospital teams and community partners to ensure appropriate services and optimal outcomes.
• Provide clinical support and appropriate referrals for chemical dependency and mental health needs.
• Advocate for clients and assist with complex matters such as Power of Attorney when relevant.
• Maintain accurate, timely, and compliant documentation (screenings, treatment plans, progress notes, required reports) to meet state, federal, accreditation and hospital billing standards.
• Participate in multidisciplinary team meetings and contribute to treatment planning and quality improvement activities.
• Assist with assigned billing duties and provider enrollment documentation as requested.

Qualifications

• Masters degree in Social Work, Psychology, Counseling, or a related discipline (required).
• Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT) — or in the process of becoming licensed.
• Valid State of Alaska driver’s license or ability to obtain one within 6 months of hire.
• Must be 21 years of age.
• Clinical competency required within 3 months of hire and every three years (SEARHC clinical competency assessment).
• Experience with discharge planning, care coordination, case management, and clinical documentation.
• Ability to complete timely and compliant charting to meet hospital billing requirements.

Education

• Masters Degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution

Knowledge & Skills

• Proficient in clinical assessment, screening, diagnosis, and treatment planning.
• Skilled in crisis intervention and interviewing techniques.
• Knowledge of discharge planning processes and transitional care placements.
• Familiarity with documentation regulations, accreditation standards, and hospital billing requirements.
• Ability to work effectively in a multidisciplinary team and coordinate with community resources.
• Competence in client advocacy, skill-building, and working with chemical dependency and mental health needs.
