Roles & Responsibilities

• Deliver high-quality, patient-centered physical therapy in the home health setting
• Perform thorough evaluations, create individualized treatment plans, and document progress in EPIC
• Collaborate with interdisciplinary teams to coordinate care and achieve patient goals
• Educate patients and caregivers to promote safety, independence, and adherence
• Ensure compliance with agency competency requirements and licensing regulations
• Participate in holiday rotation as scheduled while maintaining consistent weekday coverage

Qualifications

• Current, unrestricted PT license (license/certificate verification required)
• BLS (Basic Life Support) certification
• Experience or comfort using EPIC for clinical documentation
• Willingness to complete Trinity Skills Checklist and specialty competency testing
• Up-to-date immunizations and required health screenings (COVID-19, MMR, Hep B, Tdap, Varicella, Influenza)
• Trinity 10-panel drug screen and color vision/respirator/fit testing as required

Knowledge & Skills

• Strong assessment and therapeutic intervention skills in home health Physical Therapy
• Proficient clinical documentation skills (EPIC experience preferred)
• Ability to develop and adjust care plans based on patient response
• Excellent communication and patient/caregiver education skills
• Familiarity with home safety evaluations and functional mobility training
• Comfort navigating required competency/testing workflows (Trinity processes)
