Oncology Clinical Pharmacist

Roles & Responsibilities
- Collaborate with oncologists and the interdisciplinary team to optimize chemotherapy and supportive care regimens for inpatient and outpatient oncology patients
- Round with Oncology Fellows, NPs, and physicians to recommend evidence-based medication therapy adjustments, monitoring, and dose modifications
- Serve as the oncology subject matter expert: respond to drug information queries and ensure EHR care plans align with national guidelines
- Provide patient-specific medication education (teach-back method) for new oral oncolytics and IV chemotherapy when needed
- Establish and advance collaborative practice initiatives to improve supportive care and medication safety across the oncology service line
- Develop and deliver staff development and educational programs for pharmacy, nursing, and medical teams
- Identify and resolve medication-related problems using clinical data, labs, and patient history
- Precept pharmacy residents and students, mentoring the next generation of oncology pharmacists
- Promote cost-effective therapy and lead cost-avoidance initiatives within the Oncology Service Line
- Participate actively in Oncology Service Line meetings to enhance safety, quality, and efficacy of cancer care
Qualifications
- Doctor of Pharmacy (PharmD) required
- Active Pharmacist license (state licensure required or eligibility to obtain)
- Board Certified Oncology Pharmacist (BCOP) — listed as a credential requirement
- Minimum of three years of hospital clinical pharmacy experience and three years of oncology practice/training (per job posting)
- PGY-2 ASHP-accredited Oncology residency preferred; PGY-1 plus a minimum of 3 years oncology experience and BPS oncology certification also preferred
- Demonstrated experience with chemotherapy, oral oncolytics, supportive care management, and EHR medication management
- Strong precepting or teaching experience with trainees (residents/students) preferred
Education
- Doctorate Degree in Pharmacy (PharmD)
- PGY-1 residency or equivalent clinical pharmacy experience (minimum ~3 years) OR PGY-2 residency in Oncology (preferred/strongly desired)
Knowledge & Skills
- Deep knowledge of oncology therapeutics, chemotherapy regimens, and oral oncolytics
- Clinical pharmacy skills: therapeutic monitoring, dose adjustment, and medication safety
- Experience with Electronic Health Record (EHR) review and care plan optimization
- Patient counseling and teach-back education for complex oncology medications
- Ability to provide timely, evidence-based drug information to clinical teams
- Skill in establishing collaborative practice agreements and leading interdisciplinary initiatives
- Focus on cost avoidance, formulary stewardship, and value-based medication decisions
- Precepting and mentorship skills for residents and students
