Roles & Responsibilities

• Provide safe, patient-centered medication order entry and verification for inpatient populations
• Perform medication checking and dispensing with rigorous attention to medication safety
• Deliver comprehensive clinical pharmacy services across geriatrics to pediatrics
• Rotate into ambulatory clinics (e.g., anticoagulation, oncology) as assigned to expand outpatient care skills
• Collaborate proactively with providers, nurses, pharmacy techs and interdisciplinary teams to optimize therapy
• Ensure compliance with state and federal pharmacy laws and institutional policies
• Contribute to quality improvement efforts and medication-safety initiatives

Qualifications

• Current Pennsylvania Pharmacist License or eligible for reciprocation within 90 days of hire
• Bachelor of Science in Pharmacy required (Doctor of Pharmacy preferred)
• Inpatient hospital pharmacy experience required
• Pharmacy residency experience preferred for ambulatory rotations
• Advanced practice certifications preferred (where applicable)
• BLS (AHA Healthcare Provider) required if rotating to anticoagulation clinic
• Retail pharmacy experience will not be considered

Education

• Bachelor of Science in Pharmacy (required)
• Doctor of Pharmacy (PharmD) preferred

Knowledge & Skills

• Strong knowledge of medication safety, order entry and verification processes
• Familiarity with acute care pharmacy workflows and ambulatory pharmacy services
• Understanding of state and federal pharmacy regulations and best practices
• Excellent communication and teamwork skills with clinicians and support staff
• Ability to manage multiple patient populations and prioritize clinical tasks
• Experience with anticoagulation and oncology clinic processes is a plus
