Occupational Therapist - SNF

Shift Type: Full-time hours preferred (specific start/end times to be determined by facility); PRN (as-needed) option with flexible scheduling and opportunity for ongoing shifts
Candidate Pay Rate: $90 - $63 per hour

Roles & Responsibilities
- Evaluate residents and develop individualized treatment plans that promote independence in daily activities
- Provide skilled occupational therapy interventions in accordance with physician orders
- Target improvements in ADLs, mobility, strength, coordination, and cognitive skills
- Document evaluations, progress notes, and discharge summaries in compliance with facility and regulatory guidelines
- Collaborate with an interdisciplinary team including PT, SLP, nursing, and physicians to ensure coordinated care
- Educate patients and families on treatment plans and home exercise programs to support recovery
- Ensure compliance with state, federal, and facility regulations while delivering high-quality care
Qualifications
- Active California Occupational Therapist license (required)
- Graduate of an accredited Occupational Therapy program
- Current CPR/BLS certification
- SNF experience preferred
- Strong clinical reasoning and documentation skills
- Ability to work independently and as part of a multidisciplinary team
Education
- Graduate of an accredited Occupational Therapy program
Knowledge & Skills
- Skilled in occupational therapy evaluation and treatment planning
- Clinical reasoning and evidence-based decision making
- Strong documentation and charting abilities
- Effective communication and teamwork with interdisciplinary staff
- Knowledge of ADLs, mobility training, and cognitive interventions
- Familiarity with SNF regulatory and compliance requirements
