Location: Woodland, CA

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with rotating Saturdays once a month

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive physical therapy assessments and develop personalized treatment plans
Educate patients and families on therapeutic exercises and home safety
Collaborate with physicians and interdisciplinary team to optimize patient outcomes
Document patient progress and adjust treatment plans as needed
Supervise home health aides and coordinate care with other services
Participate in case conferences and peer consultation to enhance quality of care

Qualifications

Minimum of 2 years of experience as a Physical Therapist in acute care, home health, or rehab setting
Valid BLS certification from the American Heart Association
Strong communication and organizational skills to thrive in a home health environment
Ability to provide compassionate, patient-centered care in a variety of home settings
Willingness to travel within the Yolo County service area

Knowledge & Skills

Expertise in patient assessment, treatment planning, and gait training
Proficient in documenting progress and using electronic health records
Strong interpersonal skills to build rapport with patients and families
Knowledge of Medicare Conditions of Participation and regulatory requirements
Ability to work independently and manage a caseload efficiently
