Contract Type: Travel Contract

Shift Type: Evening shift, 2:30 PM - 11:00 PM, 32 hours per week, includes every other weekend and other shifts as needed

Hours per Week: 32

Candidate Pay Rate: $63 per hour

Roles & Responsibilities

Provide leadership and vision as a role model for professional nursing practices in the household
Ensure clinical operations comply with federal, state, and local regulations and person-centered care standards
Administer medications, observe residents' conditions, and communicate changes to the RN or physician
Collaborate with interdisciplinary teams to develop and implement individualized care plans
Train and mentor household staff on clinical competencies and professional standards
Foster a welcoming, home-like environment that promotes residents' well-being and independence

Qualifications

Active and unrestricted LPN license in Minnesota
Experience in long-term care or skilled nursing preferred
Certifications such as CPI, ENPC, TNCC, or AWHONN are a plus
Strong clinical documentation and medication administration skills
Commitment to person-centered and resident-directed care
Ability to work evening shifts and every other weekend
