Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM; rotating Saturdays once a month

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in a home health setting
Develop and implement therapeutic plans of care based on physician orders
Instruct patients and families in the use of therapeutic appliances and home exercise programs
Monitor patient progress and adjust treatment plans as needed
Collaborate with interdisciplinary team members to ensure coordinated care
Document clinical findings and progress summaries accurately and timely

Qualifications

Minimum of 2 years of experience as a Physical Therapist in acute, home care, or rehab setting
Valid BLS certification required
Active state license to practice physical therapy in California
Excellent communication and organizational skills
Ability to work independently and manage a geographically dispersed caseload

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Knowledge of home health regulations and Medicare conditions of participation
Proficient in documenting patient care and progress
Ability to educate patients and families on therapeutic interventions
Time management and scheduling flexibility
