Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to home health patients in the Valley City area
Develop and implement individualized treatment plans to restore function and independence
Educate patients and caregivers on home exercise programs and safety strategies
Coordinate care with physicians, nurses, and other members of the interdisciplinary team
Document patient progress accurately in the HomeCare/HomeBase EMR system
Travel within a 60-mile radius to deliver care in patients' homes

Qualifications

Current North Dakota Physical Therapy license in good standing
BLS certification from the American Heart Association
Valid driver's license and auto insurance with reliable transportation
Minimum 1 year of physical therapy experience, preferably in home health
Willingness to travel up to 60 miles for patient visits

Knowledge & Skills

Proficient in home health physical therapy assessment and treatment techniques
Experience with HomeCare/HomeBase or similar EMR systems
Excellent communication and patient education skills
Ability to manage a caseload of 5-7 patients per day efficiently
Knowledge of fall prevention, post-surgical rehab, and chronic disease management
