Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct start-of-care, re-certifications, re-evaluations, and daily visits for home health patients
Develop and implement individualized physical therapy treatment plans
Educate patients and families on therapeutic exercises and home safety
Collaborate with clinical managers and interdisciplinary team for optimal patient outcomes
Complete accurate and timely documentation in accordance with regulations

Qualifications

Active Physical Therapist license in Kentucky or compact eligibility
BLS certification required
Home health experience preferred; new grads considered with mentorship
Ability to work independently and travel to patient homes
Reliable transportation and valid driver's license

Knowledge & Skills

Knowledge of OASIS documentation and home health regulations
Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities
Time management and adaptability in a fieldwork environment
