Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct start-of-care visits and develop individualized treatment plans for orthopedic and geriatric patients
Manage a daily caseload of 5 visits, focusing on joint replacements, CHF, and COPD
Utilize HCHB (HomeCare HomeBase) for accurate documentation and patient tracking
Collaborate with nursing and therapy team members to optimize patient outcomes
Educate patients and families on home exercise programs and safety strategies

Qualifications

Graduate of an accredited Physical Therapy program (Master's or Doctorate)
Current California PT license in good standing
BLS certification from the American Heart Association
Home Health experience required (travelers with less than 2 years considered if experienced in home health)
Proficiency with HCHB (HomeCare HomeBase) EMR system preferred

Education

Master's or Doctorate in Physical Therapy from an accredited institution

Knowledge & Skills

Strong clinical assessment and treatment skills for orthopedic and geriatric populations
Excellent communication and patient education abilities
Independent problem-solving and time management in a home health environment
Familiarity with OASIS documentation and SOC processes
