Roles & Responsibilities
- Coordinate patient care and discharge planning across departments
- Conduct comprehensive psychosocial assessments and interventions
- Provide crisis intervention and facilitate 5150 evaluations for patients in psychiatric need
- Collaborate with an interdisciplinary team to ensure holistic patient care
- Advocate for patient needs and connect them with community resources
Qualifications
- Active LCSW license in California
- Current BLS certification
- 5150 certification (completed training and passed exam with 80%)
- Prior experience with CommonSpirit/CHI/Dignity Health facilities
- Two supervisory references, one within 12 months and one within 3 years
Knowledge & Skills
- Expertise in care coordination and social work practices
- Knowledge of 5150 involuntary hold process and mental health crisis intervention
- Strong communication and interdisciplinary collaboration skills
- Ability to thrive in a fast-paced healthcare environment
