Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments in home health settings
Develop and implement individualized treatment plans to improve mobility and function
Collaborate with nurses, occupational therapists, and speech therapists for holistic care
Educate patients and families on exercises and safety strategies
Document patient progress in HCHB (HomeCare HomeBase) EMR system
Travel within Tehama County to provide care across the service area

Qualifications

Current California Physical Therapy license in good standing
Basic Life Support (BLS) certification required
Home health experience strongly preferred; travelers with less than 2 years will be considered if they have home health background
Ability to drive up to 40 miles per day within the service area
Comfortable using tablet-based EMR (HCHB) for documentation

Knowledge & Skills

Strong understanding of orthopedic and geriatric conditions (e.g., joint replacements, CHF, COPD)
Excellent communication and patient education skills
Time management and ability to prioritize a daily caseload of 5 visits
Knowledge of home health regulations and Medicare guidelines
Adaptability to work independently in a home environment
