Shift Type: Day shifts, typically Monday-Friday, no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive physical therapy evaluations for home health patients
Develop and implement individualized treatment plans focused on functional improvement
Perform SOC visits for ortho patients when nursing is not ordered
Collaborate with interdisciplinary team to coordinate patient care
Document patient progress accurately using HCHB EMR system

Qualifications

Current Physical Therapist license in California
Basic Life Support (BLS) certification
Home health experience required (new graduates considered with strong clinical rotation)
Ability to travel within Tehama County service area (approx. 40 miles/day)

Knowledge & Skills

Proficiency in HomeCare HomeBase (HCHB) EMR
Experience treating orthopedic and geriatric patients with comorbidities like CHF and COPD
Strong clinical reasoning and time management skills
Excellent communication and patient education abilities
