Location: Louisville, KY

Start Date: 06-29-2026

End Date: 09-28-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating weekend shifts and holiday rotation

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive start of care assessments for home health patients
Perform recertifications and re-evaluations to ensure ongoing care needs
Develop and implement personalized treatment plans
Provide skilled physical therapy visits in patients' homes
Collaborate with clinical managers and educators for optimal patient outcomes
Document patient progress and communicate with interdisciplinary team

Qualifications

Current Physical Therapist license in Kentucky (or compact if applicable)
BLS certification required
Prefer experience in home health, but new grads are welcome to apply
Ability to work rotating weekends and holidays
Must have reliable transportation and a willingness to travel within assigned areas

Knowledge & Skills

Expertise in start of care, recertifications, and evaluations
Strong clinical reasoning and problem-solving skills
Excellent communication and patient education abilities
Comfortable using provided tablet and phone for documentation
Ability to work independently and as part of a team
