Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in a home health setting across four rural counties
Conduct Oasis Assessments and Start of Care cases for new patients
Develop and implement individualized treatment plans for elderly patients recovering from falls or fractures
Utilize a point system to manage an equivalent of 4-6 patient visits per day
Track time and visits using a tablet device
Collaborate with healthcare team to ensure comprehensive patient care

Qualifications

Active Physical Therapist (PT) license in the state of Indiana
Experience with Oasis Assessments and Start of Care documentation
Comfortable working in a rural setting with travel between multiple counties
Ability to manage a caseload of 4-6 visits per day efficiently

Knowledge & Skills

In-depth knowledge of home health therapy protocols and Medicare guidelines
Strong assessment and clinical reasoning skills for falls and fractures
Proficient in using electronic tablets for documentation
Excellent communication and patient education abilities
