Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care, recertifications, and resumption of care visits for home health patients
Manage a daily caseload of 5-6 visits or up to 6.5 points per day
Perform admissions and OASIS assessments as needed
Collaborate with interdisciplinary team to ensure quality patient care
Document patient progress and adjust treatment plans accordingly

Qualifications

Current and active North Dakota Physical Therapy License
Basic Life Support (BLS) certification
Valid Driver's License and auto insurance
Home health experience preferred
OASIS training or experience
First-time travelers welcome to apply

Knowledge & Skills

Expertise in home health and hospice therapy
Proficient in OASIS documentation
Strong clinical assessment and planning skills
Excellent communication and patient education
Ability to work independently and manage a schedule
