Roles & Responsibilities

• Provide compassionate, culturally sensitive diabetes education to patients and families as part of a multidisciplinary diabetes management team
• Assess patients, develop individualized care plans, and document clinical findings according to standards of care
• Administer prescribed medications and treatments safely and in line with nursing best practices and state regulations
• Act as an educational resource for staff, contributing to patient education materials and programs
• Collaborate with interdisciplinary team members to support the patient’s longitudinal plan of care
• Use critical thinking to identify patient safety concerns and escalate appropriately
• Participate in department performance improvement activities and shared governance initiatives
• Float to other units as needed based on clinical competency and departmental needs

Qualifications

• Licensed as a Registered Nurse (RN) in Illinois and Wisconsin (required)
• Graduate of an accredited nursing program (required); BSN preferred
• CDCES (Certified Diabetes Care and Education Specialist) required
• Approved CPR certification required within 90 days
• Comfort working within interdisciplinary teams and serving as a patient education resource
• Willingness to float to other units and participate in on-call/holiday coverage as needed

Education

• Graduate of an accredited nursing program
• BSN preferred
