Roles & Responsibilities

• Perform comprehensive patient assessments and monitor telemetry for early detection of changes
• Provide skilled wound care and advanced treatment interventions as required
• Administer medications safely and document care accurately in Cerner
• Collaborate with interdisciplinary teams to create patient-centered discharge and rehab plans
• Deliver concierge-level patient experience for an affluent community
• Perform minor procedures on the unit and ensure proper post-procedure care
• Educate patients and families about care plans and transitions to outpatient or swing bed services

Qualifications

• Active Registered Nurse (RN) licensure as required by the state
• Minimum 2 years clinical nursing experience (telemetry or med/surg experience preferred)
• Must have experience caring for geriatric patients or past inpatient rehab (swing bed) experience
• BLS certification required
• ACLS and CPI preferred
• Demonstrated competency with wound care and minor procedures
• Experience with Cerner documentation preferred

Knowledge & Skills

• Telemetry monitoring and cardiac rhythm interpretation
• Med/Surg nursing best practices
• Advanced wound care techniques
• Electronic charting with Cerner
• Comfort working with higher patient ratios and limited ancillary support
• Strong communication and patient/family education skills
