Roles & Responsibilities

• Provide one-on-one physical therapy assessments and develop individualized care plans for homebound patients
• Deliver therapeutic interventions and mobility training that improve safety and independence
• Educate patients and caregivers on home exercise programs and fall-prevention strategies
• Collaborate with nurses, social workers, and primary care teams to coordinate holistic care
• Document patient progress clearly and timely to support outcomes and reimbursement
• Adjust treatment plans based on functional gains, barriers, and patient goals

Qualifications

• Current state Physical Therapist license in the state of practice or eligible to obtain
• Doctor of Physical Therapy (DPT) or Master of Physical Therapy preferred
• Home health or outpatient experience preferred; new grads with strong clinical skills considered
• Current Basic Life Support (BLS) certification
• Valid driver license and reliable transportation for home visits
• Strong clinical reasoning, excellent communication, and a patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred; Master of Physical Therapy accepted
• Bachelor's degree in related field considered with relevant clinical experience

Knowledge & Skills

• Skilled in functional mobility assessment, gait training, and ADL-focused interventions
• Comfortable with documentation systems and outcome measure reporting
• Knowledge of home safety assessments and adaptive equipment recommendations
• Ability to prioritize caseloads, manage time, and work independently in community settings
• Strong interpersonal skills for coaching patients and collaborating with multidisciplinary teams
