Roles & Responsibilities

• Conduct thorough home health physical therapy evaluations and develop individualized treatment plans
• Deliver skilled therapeutic interventions to improve mobility, function, and safety in the home environment
• Document patient visits and progress accurately in the HCHB electronic documentation system
• Coordinate care with nurses, social workers, and other interdisciplinary team members to optimize outcomes
• Provide patient and caregiver education to support safe function and reduce hospital readmissions
• Manage a day-to-day caseload efficiently while maintaining high clinical standards and compassionate care
• Ensure infection control, safety, and professional standards during home visits

Qualifications

• Current, unrestricted Physical Therapist license for the state of practice (North Carolina) required
• Minimum 2 years of clinical experience, with home health experience preferred
• Proven experience with home health patient evaluations, care planning, and skilled interventions
• Comfortable with home visit logistics — reliable vehicle and valid driver’s license required
• Strong communication skills and ability to collaborate with interdisciplinary teams
• Familiarity with hospice care a plus (nursing roles require hospice/home health experience; therapy roles require home health experience)

Education

• Doctor of Physical Therapy (DPT) or equivalent physical therapy degree as required by state licensing

Knowledge & Skills

• Proficient in HCHB or similar home health documentation systems
• Skilled in assessment of mobility, ADL/IADL needs, balance, transfer training, and gait training
• Ability to prioritize caseload and manage time effectively for multiple daily home visits
• Knowledge of home safety assessments and adaptive equipment recommendations
• Patient-centered teaching skills for caregivers and family members
