Roles & Responsibilities

• Deliver compassionate, patient-centered physical therapy services as a licensed PTA in a skilled nursing facility
• Implement treatment plans focused on mobility, balance, transfers, and safe functional activity
• Document patient progress clearly and timely to support quality care and interdisciplinary communication
• Collaborate with therapists, nurses, and support staff to ensure coordinated care and positive outcomes
• Coach patients and families on home programs and fall prevention to promote independence
• Adapt treatments to meet the needs of an SNF population and support therapy goals

Qualifications

• Licensed or certified PTA in New Hampshire, or eligible to obtain state licensure
• Any experience level considered, including new grads (job explicitly welcomes new graduates)
• Comfort working in a skilled nursing facility (SNF) setting
• Strong communication and team collaboration skills
• Current CPR/BLS preferred or willingness to obtain upon hire

Education

• Associate degree in Physical Therapist Assistant preferred
• New graduates welcome as years of experience requirement states 'any, including New Grad'

Knowledge & Skills

• Knowledge of therapeutic exercise, gait training, transfers, and assistive devices
• Skilled in documentation and using electronic medical records for therapy notes
• Familiarity with SNF workflows, discharge planning, and interdisciplinary team meetings
• Ability to assess functional mobility and set measurable therapy goals
• Good clinical judgment, time management, and patient education skills
• Compassionate bedside manner and ability to motivate patients of varying ability levels
