Roles & Responsibilities

Provide compassionate, patient-centered care to residents in a long-term care setting
Assess patient conditions, develop care plans, and coordinate with interdisciplinary teams
Administer medications and treatments as prescribed, ensuring safety and efficacy
Monitor vital signs and respond to changes in patient status promptly
Educate patients and families on health management and discharge planning
Maintain accurate documentation and uphold facility policies and regulatory standards

Qualifications

Active Registered Nurse (RN) license in West Virginia
Basic Life Support (BLS) certification from the American Heart Association
Strong clinical assessment and critical thinking skills
Excellent communication and teamwork abilities

Knowledge & Skills

Knowledge of nursing principles and practices in long-term care
Proficiency in electronic health records (EHR) systems
Ability to prioritize and manage multiple patient needs
Compassionate and patient-focused approach to care
