Roles & Responsibilities

• Provide compassionate and skilled physical therapy services to patients in a transitional care setting
• Develop individualized treatment plans to improve mobility, strength, and functional independence
• Collaborate with interdisciplinary teams to ensure holistic patient care
• Educate patients and families on exercises and techniques for continued progress
• Document patient progress accurately and maintain compliance with regulatory standards

Qualifications

• Active Physical Therapist license in New Mexico (or compact privilege if applicable)
• Graduate of an accredited physical therapy program (DPT preferred)
• BLS certification from the American Heart Association
• Minimum of 1 year of recent experience in a similar setting preferred
• Strong interpersonal and communication skills to connect with patients and team members

Knowledge & Skills

• Expertise in therapeutic exercises, manual therapy, and modality application
• Knowledge of geriatric rehabilitation and transitional care best practices
• Ability to use electronic medical records (EMR) systems efficiently
• Time management and organizational skills to handle a patient caseload
• Problem-solving skills to adapt treatment plans as needed
