Location: Breckenridge, MN

Start Date: 07-06-2026

End Date: 10-05-2026

Contract Type: Travel Contract

Shift Type: Rotating shifts: 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, 10:00 PM - 6:30 AM; every other weekend and holiday required

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide compassionate direct patient care in a long-term care setting
Administer medications and treatments as prescribed, ensuring accuracy and safety
Monitor and document vital signs, changes in condition, and patient responses
Collaborate with RNs, CNAs, and interdisciplinary team to develop and implement care plans
Use PointClickCare EMR for accurate and timely documentation
Support a home-like environment through strong teamwork and communication

Qualifications

Active LPN license in Minnesota (or compact state)
Current BLS certification (AHA required)
Experience with PointClickCare EMR preferred
Previous long-term care experience a plus
Compassionate, reliable, and team-oriented attitude

Knowledge & Skills

Knowledge of geriatric nursing and long-term care best practices
Proficiency in medication administration and IV therapy (minimal IVs)
Strong assessment and documentation skills
Ability to work effectively in a team environment
Adaptability to rotating shifts and every other weekend/holiday
