Roles & Responsibilities

• Lead and coordinate the Long-Term Care QAPI program to improve clinical outcomes and resident safety
• Plan, organize, and direct quality assurance activities and accreditation readiness across the facility
• Facilitate regulatory surveys and maintain ongoing compliance with CMS and other regulatory bodies
• Conduct chart audits, data analysis, and performance reviews to identify improvement opportunities
• Develop, revise, and implement policies, procedures, and action plans to address identified deficiencies
• Coordinate RCA (root cause analysis) and investigations of incidents, complaints, and facility-reported events
• Organize and facilitate quality committee meetings, track recommendations, and maintain meeting minutes
• Coach and educate staff, leaders, and providers to integrate quality and patient safety into daily operations

Qualifications

• Active, unrestricted Registered Nurse (RN) license in Alaska (agency staff must have active license; direct hires must have applied before start)
• Certified Professional in Healthcare Quality (CPHQ), QAPI Certified Professional (QCP), or other LTC Quality Certification obtained within the last 3 years
• Basic Life Support (BLS) certification (American Heart Association)
• Clinical Competency Assessment - SEARHC (required)
• Minimum 2 years of experience monitoring quality and regulatory requirements, planning and coordinating process changes
• Demonstrated leadership in facilitating cross-functional teams and working collaboratively across disciplines
• Willingness and ability to travel at least 30% for facility visits and accreditation activities

Knowledge & Skills

• Strong knowledge of accreditation requirements across long-term care, acute care, ambulatory and behavioral health
• Experience with data analytics: collecting, analyzing, and presenting clinical and quality data
• Skilled in chart audits, chart review, and extraction of clinical data
• Proficient with Microsoft Word, Excel, and PowerPoint
• Infection control facilitation and patient safety best practices
• Familiarity with Datix or similar incident reporting systems and root cause analysis methods
• Excellent verbal and written communication, coaching, and facilitation skills
