Roles & Responsibilities

• Evaluate patients and create individualized, goal-driven treatment plans
• Implement evidence-based therapeutic interventions to improve mobility and function
• Document clinical progress accurately and timely following facility and CRG policies
• Collaborate with nursing, rehab aides, and interdisciplinary team members to coordinate care
• Maintain productivity expectations and contribute to team performance (90% productivity requirement)
• Adhere to facility dress code and professional standards, including black scrubs
• Be present onsite during scheduled hours to ensure continuity of care and guaranteed hours

Qualifications

• Licensed Physical Therapist (PT) — active licensure required; DPT strongly preferred
• 1-3 years of clinical experience (experience in skilled nursing facility or post-acute care is a strong fit)
• Ability to meet productivity expectations and follow CRG policies and procedures
• Strong communication skills and comfort working within an interdisciplinary team
• Availability for a 13-week contract beginning 2025-11-24

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent Physical Therapy degree
• Active PT license in the state of New Mexico

Knowledge & Skills

• Skilled in patient evaluation, functional mobility training, and therapeutic exercise
• Knowledge of documentation best practices in SNF/post-acute settings
• Experience with care planning for geriatric and post-acute populations
• Solid teamwork, time management, and patient-centered communication skills
• Familiarity with productivity tracking and meeting contractual performance metrics
