Roles & Responsibilities
- Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care using evidence-based assessments
- Develop individualized plans of care that meet regulatory and clinical practice standards
- Deliver therapeutic interventions including manual techniques, ambulation training, therapeutic exercises, modalities, and appropriate use of assistive devices
- Provide wound care and monitor healing, adjusting treatment plans as outcomes indicate
- Document thoroughly in the electronic documentation system, including treatment minutes and discharge summaries per regulatory requirements
- Communicate effectively with supervisors, interdisciplinary team members, patients, and families about progress and barriers
- Participate in care conferences, utilization review, and facility-required meetings and trainings
- Supervise and mentor physical therapist assistants, aides, and students, ensuring quality and compliance
Qualifications
- Missouri State Physical Therapy license (MO) — required; Compact allowed
- Minimum 1 year of PT experience (new graduates OK with Level II clinical experience)
- Prior LTC (Long Term Care) or Rehab experience — required
- Ability to perform standardized assessments and apply evidence-based practice
- Comfort supervising PT assistants and supporting rehabilitation aides/students
- Adherence to APTA and State Practice Act requirements, professional code of conduct, and ethics
- Ability to obtain necessary DME and supplies per practice requirements
Knowledge & Skills
- Patient evaluation and assessment using standardized tools
- Plan of care development and outcome evaluation
- Rehabilitation interventions: manual techniques, ambulation, therapeutic exercises, modalities
- Wound care assessment and treatment
- Clinical documentation and electronic documentation systems
- Regulatory compliance and accurate recording of treatment minutes
- Supervision, mentoring, and team-based communication
- Experience in LTC and rehabilitation settings
