Roles & Responsibilities
- Provide compassionate direct patient care with attention to safety and dignity
- Administer medications and treatments accurately according to policy
- Perform timely and thorough patient assessments and document findings
- Collaborate with the care team to develop and update patient plans
- Monitor patient status and escalate changes to RN or provider as needed
- Support smooth shift handoffs with clear, concise communication
Qualifications
- Current Licensed Practical Nurse (LPN) license (New Mexico preferred)
- BLS/CPR certification or willingness to obtain promptly
- Previous LPN experience in a long term care or similar clinical setting preferred
- Ability to work nights and weekend shifts reliably
- Good communication skills and a patient-centered approach
Education
- LPN / Vocational Nursing diploma or certificate
Knowledge & Skills
- Strong clinical assessment and basic nursing skills
- Medication administration and safe documentation practices
- Familiarity with electronic medical records and charting
- Infection control, wound care basics, and vital sign monitoring
- Time management, teamwork, and clear clinical communication
