Shift Type: Tuesday-Saturday, 8:30 AM - 5:00 PM (Day shift) or Sunday-Thursday, 8:30 AM - 5:00 PM (Day shift); on-call duties and floating between units required
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Conduct thorough patient assessments and document findings accurately in Epic
- Develop and implement individualized plans of care focused on recovery and function
- Provide gait training, ADL training, and use of assistive devices to maximize independence
- Deliver rehabilitation modalities such as cryotherapy and moist heat when appropriate
- Perform wheelchair assessments, prosthetic training, and safe transfer techniques including Hoyer lift use
- Reassess patients regularly, document progress, and coordinate discharge planning
- Collaborate with interdisciplinary teams including respiratory, pharmacy, radiology, and social services to optimize outcomes
- Float between acute care and acute rehab units as needed and respond to on-call requests
Qualifications
- Minimum 1 year of acute care hospital experience (explicitly noted)
- Active state physical therapy license required (pending licenses are not accepted)
- BLS certification preferred
- Experience charting in Epic is required
- Comfortable floating between like units and responding to on-call duties
- Trauma Level II experience preferred but not required
Knowledge & Skills
- Epic charting and accurate documentation practices
- Patient assessment, plan of care development, and reassessments
- Gait training, ADL training, and use of adaptive devices
- Prosthetic training and wheelchair assessment skills
- Hands-on experience with modalities such as cryotherapy and moist heat
- Safe lifting and transfer techniques including Hoyer lift and sit-to-stand devices
- Familiarity with billing populations: Medicare A, Medicare B, Medicaid, and commercial
- Ability to work with adult and geriatric patient populations and neurological, cardiopulmonary, oncology, and orthopedic cases
