Roles & Responsibilities
- Provide compassionate, patient-centered nursing care following care plans and facility policy
- Administer medications and document patient responses accurately and timely
- Monitor vital signs, assess patient status, and escalate changes to the care team
- Support patient activities of daily living with dignity and respect
- Collaborate with RNs, providers, and interdisciplinary staff to coordinate excellent care
- Maintain clear, professional documentation and communicate effectively with families
- Participate in quality improvement initiatives and safety practices to enhance patient outcomes
Qualifications
- Current Licensed Practical Nurse (LPN) license — Pennsylvania license preferred or eligible
- Basic Life Support (BLS) certification required; additional certifications welcomed
- Prior bedside nursing experience preferred, especially in long-term care or acute settings
- Strong communication skills and a patient-first attitude
- Ability to work collaboratively in a fast-paced healthcare environment
Education
- Vocational nursing diploma or equivalent nursing education required
- Completion of an accredited LPN program
Knowledge & Skills
- Solid clinical assessment and medication administration skills
- Comfortable with electronic medical records and accurate documentation
- Infection control practices and safety-minded approach
- Time management and prioritization to handle varying patient needs
- Empathy, professionalism, and strong teamwork abilities
