Roles & Responsibilities

• Provide comprehensive physical therapy services to home health clients, focusing on restoring function and improving quality of life
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Conduct thorough patient assessments, including OASIS documentation, to track progress and adjust treatments
• Educate patients and families on therapeutic exercises, modalities, and adaptive equipment use
• Maintain accurate clinical records and communicate effectively with physicians and other healthcare providers

Qualifications

• Active Massachusetts Physical Therapist License (must be uploaded with profile)
• Active American Heart Association BLS certification
• Minimum 2 years of acute nursing care (hospital-based) and home health experience
• Proven proficiency with OASIS documentation (non-negotiable)
• Valid driver's license and reliable vehicle for travel to patient homes
• Ability to work independently with minimal supervision
• Traveler must reside more than 50 miles from the facility

Knowledge & Skills

• Expertise in home health physical therapy and OASIS
• Strong clinical assessment and critical thinking abilities
• Excellent communication and patient education skills
• Proficient with rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, and ADL equipment
• Familiarity with acute care settings (preferred)
