Roles & Responsibilities

• Evaluate and treat skilled nursing facility patients using evidence-based occupational therapy approaches
• Collaborate with the interdisciplinary team to develop and update care plans that promote patient progress
• Complete timely, accurate documentation in Net Health according to facility standards
• Manage caseload effectively with strong time-management skills to maximize patient outcomes
• Provide mentorship and support to newer clinicians when assigned, helping foster a strong learning culture
• Float to a nearby sister facility as needed to ensure continuity of care across sites

Qualifications

• Active California Occupational Therapy license (or license in process under the 60-day rule)
• SNF (skilled nursing facility) experience preferred, though new graduates are welcome
• Comfortable and accurate documentation in Net Health or similar EMR systems
• Strong time-management skills and ability to manage a dynamic caseload
• Flexible, collaborative mindset and ability to work effectively with interdisciplinary teams
• Willingness to float to a sister facility within walking distance when required

Education

• Professional degree in Occupational Therapy (e.g., MOT, OTR, OTD) and state licensure

Knowledge & Skills

• Skilled in occupational therapy evaluation and treatment for geriatric/SNF populations
• Knowledge of SNF regulatory standards and documentation requirements
• Proficiency with Net Health documentation or similar rehab EMRs
• Ability to collaborate with PT, SLP, nursing, social work, and other disciplines
• Good clinical judgment, prioritization, and organizational skills
