Roles & Responsibilities

Provide compassionate, patient-centered care as an LPN working within the assigned scope
Administer medications and treatments accurately and document per facility policy
Perform routine assessments and report changes in patient condition promptly
Collaborate with the interdisciplinary team to support coordinated care plans
Maintain clear, timely charting and complete required documentation
Support patient comfort, dignity, and education during care activities

Qualifications

Active Licensed Practical Nurse (LPN/LVN) license in Pennsylvania (required)
Current Basic Life Support (BLS) / CPR certification (required or obtained upon hire)
Experience in a clinical setting preferred — long-term care, skilled nursing, or similar experience is a plus
Strong communication skills and ability to work well on a care team
Current immunizations and background checks as required by facility policy

Education

Completion of an accredited LPN/LVN program
High school diploma or GED

Knowledge & Skills

Medication administration and basic clinical procedures
Patient assessment and vital sign monitoring
Charting and electronic health record (EHR) documentation
Infection control and standard precautions
Effective communication with patients, families, and clinical teams
Time management and prioritization in a fast-paced environment
