Roles & Responsibilities

Provide compassionate, hands-on personal care to residents and patients
Assist with activities of daily living including bathing, dressing and mobility
Take and record vital signs accurately and consistently
Observe and report changes in patient condition to nursing staff
Support safe and dignified transfers and ambulation
Maintain clean, safe patient environments and follow infection-control practices
Document care in charts or electronic records in a timely manner
Work with the care team to deliver individualized patient-centered care

Qualifications

Current CNA certification in Pennsylvania (required)
Previous experience in long-term care, skilled nursing, or similar setting preferred
Current CPR/BLS certification preferred or willingness to obtain
Up-to-date immunizations and TB screen per facility policy
Able to pass background and health screenings as required by the facility
Dependable, punctual, and able to lift/transfers per facility guidelines

Education

High school diploma or equivalent
Completion of an accredited CNA program and current state certification

Knowledge & Skills

Strong patient care and bedside skills
Accurate vital sign measurement and documentation
Basic infection control and safety practices
Effective communication with patients, families and care teams
Teamwork and flexibility in a fast-paced care environment
Familiarity with electronic charting or willingness to learn
