Roles & Responsibilities

• Coordinate patient care across the hospital as the Care Coordinator — keep care flowing and patients supported
• Manage a daily caseload (typically 15–25 patients) with clinical judgment and efficient prioritization
• Document accurately in Cerner and maintain up-to-date case records
• Collaborate with interdisciplinary teams — physicians, social work, discharge planners and unit staff — to facilitate safe transitions
• Provide patient and family education and support for discharge planning and follow-up
• Float to various units (excluding surgery) as needed to ensure continuity of care
• Participate in orientation and mentoring with the RNCC to maintain unit standards and best practices

Qualifications

• Active California Registered Nurse (RN) license required
• BLS certification required
• Minimum 2 years of RN experience (as listed) with recent case management/care coordination experience
• The Director requires recent experience as a Care Coordinator at a California Dignity Health/CommonSpirit hospital — candidates without this will not be considered
• Two supervisor references required at submittal (charge accepted): one within the last 12 months and one within the last 3 years
• Willingness and ability to float to multiple units (first-to-float), excluding surgery
• Comfort using Cerner electronic charting and managing acute care case loads

Education

• Active California RN license
• BSN not required (BSN: No as listed); equivalent nursing education acceptable

Knowledge & Skills

• Care Coordination and Case Management best practices
• Acute care workflows and priorities
• Proficiency with Cerner charting system
• Strong clinical assessment, triage and prioritization skills
• Experience with cardiac, surgical, oncology and critical care patient populations
• Excellent verbal and written communication and teamwork abilities
• Adaptability to float across units and manage varying patient volumes
