Roles & Responsibilities

• Provide compassionate bedside care to patients, ensuring comfort and safety
• Administer medications and treatments according to facility protocols
• Monitor and document patient vital signs, changes, and responses to care
• Collaborate with RNs, providers, and the care team to support patient plans
• Maintain accurate and timely charting in the electronic medical record
• Assist with admissions, discharges, and patient education to support recovery
• Follow infection control and safety procedures to protect patients and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of practice or eligible to obtain
• Current Basic Life Support (BLS/CPR) certification preferred
• Prior clinical experience in a long-term care or rehab setting preferred
• Strong communication skills and a patient-centered attitude
• Ability to pass background checks and meet facility onboarding requirements

Education

• Graduate of an accredited Practical Nursing program or equivalent

Knowledge & Skills

• Clinical skills in medication administration and wound care
• Proficiency in patient assessment and monitoring
• Comfortable with electronic medical records and documentation
• Excellent teamwork and interpersonal communication
• Time management and the ability to prioritize in a fast-paced setting
