Shift Type: 8-hour day shifts, 40 hours guaranteed per week

Roles & Responsibilities

Provide skilled nursing care to patients in their homes
Develop and implement individualized care plans
Collaborate with healthcare providers and community representatives to coordinate care
Document patient assessments, interventions, and responses
Evaluate patient and family response to interventions and adjust care as needed
Maintain confidentiality and exercise independent judgment in clinical decision-making

Qualifications

Active Massachusetts LPN license (required)
Current BLS certification from the American Heart Association or American Red Cross (required)
Valid driver's license and reliable personal vehicle (required)
One year of acute nursing care experience (required); hospital-based experience preferred
Home health experience (required)
Certified Home Health with OASIS proficiency (highly preferred)
Infusion therapy experience (preferred)

Knowledge & Skills

Strong clinical skills with knowledge of community health principles
Ability to work independently with minimal supervision
Excellent interpersonal and communication skills
Critical thinking and problem-solving abilities
Proficiency in record keeping and documentation
Comfortable driving in inclement weather
Flexibility and adaptability in varied assignments
