Shift Type: Evening shift, 2:00 PM - 10:30 PM, includes every weekend commitment (PM shift)

Hours per Week: 40

Candidate Pay Rate: $31 per hour

Roles & Responsibilities

Provide compassionate direct patient care, including medication administration and basic nursing procedures
Perform routine patient assessments and promptly report any changes to the care team
Document patient care accurately and maintain compliance with facility policies
Assist with admissions, transfers, and discharges to ensure smooth patient transitions
Collaborate with RNs, physicians, and support staff to deliver high-quality, coordinated care
Support infection control practices and maintain a safe, clean environment for patients

Qualifications

Valid LPN license or practical nursing credentials with eligibility to work in Vermont
Comfortable committing to every weekend as required by the position
Reliable transportation, as local travel to the facility is required
Clinical experience in a long term care or similar setting preferred
Current BLS/CPR certification preferred or willing to obtain

Education

Practical Nursing diploma or completion of an accredited LPN program

Knowledge & Skills

Strong clinical skills in medication administration and basic wound care
Good assessment, observation, and critical thinking abilities
Clear communication and effective teamwork with interdisciplinary staff
Accurate documentation skills and good time management
Ability to follow policies, procedures, and safety protocols
