Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Assess patient status and monitor vital signs, promptly reporting changes
• Administer medications and treatments accurately and document appropriately
• Collaborate with the interdisciplinary care team to coordinate patient-centered care
• Educate patients and families about care plans, medication, and safety
• Maintain clear, timely and accurate clinical documentation in the charting system
• Follow infection control, safety protocols, and facility policies to protect patients and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania required
• Current CPR/BLS certification required
• Previous LPN clinical experience preferred
• Comfortable with medication administration, charting, and basic nursing procedures
• Ability to pass standard background check and drug screening

Education

• Completion of an accredited LPN program; high school diploma or equivalent

Knowledge & Skills

• Medication administration and documentation
• Patient assessment and vital sign monitoring
• Basic wound care and infection control practices
• Familiarity with electronic health record/charting systems
• Strong communication and teamwork skills
• Good time management and prioritization in a fast-paced setting
