Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and document according to facility protocol
• Perform focused patient assessments and communicate changes promptly to the care team
• Collaborate with RNs, providers, and interdisciplinary staff to deliver coordinated care
• Maintain clear, timely charting and ensure regulatory compliance with documentation
• Support residents’ activities of daily living with dignity and respect, promoting comfort and safety

Qualifications

• Current LPN licensure (practical nurse) — required
• Comfortable working the listed rotational evening/weekend schedule (3:00 PM - 11:30 PM)
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Prior experience in long-term care or similar clinical setting is a plus
• Strong communication skills and the ability to work collaboratively on a fast-paced unit

Education

• LPN diploma or practical nursing certificate

• Care planning and handoff communication
• Infection control practices and safety-focused care
• Time management during busy evening shifts
