Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living (bathing, dressing, feeding).
• Monitor and record patient vital signs and changes in condition accurately and promptly.
• Support mobility and safe transfers, using proper body mechanics and assistive devices when needed.
• Collaborate with nurses and interdisciplinary team to follow care plans and report concerns.
• Maintain clear, timely documentation in patient charts and electronic health records.
• Ensure a clean, safe environment by following infection control and safety protocols.

Qualifications

• Current LNA (Licensed Nursing Assistant) certification valid for practice in VT/NH.
• Ability to work scheduled night shifts (Tuesday, Wednesday, Thursday).
• Comfortable with travel assignments and completing facility onboarding requirements.
• Current basic life support (BLS) certification preferred or willingness to obtain.
• Proof of required immunizations and TB screening per facility policy.

Education

• LNA certification (state-specific) required
• High school diploma or equivalent preferred

Knowledge & Skills

• Strong patient care skills with a focus on dignity and respect.
• Accurate vital signs measurement and basic clinical observation.
• Effective communication with patients, families, and clinical team members.
• Familiarity with documentation best practices and basic EHR navigation.
• Time management and ability to prioritize in a fast-paced setting.
• Knowledge of infection control standards and safe patient handling techniques.
