Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living for residents
• Monitor and record vital signs, behaviors, and patient responses; promptly report changes to the care team
• Assist nursing staff with treatments and follow care plans accurately and reliably
• Maintain a safe, clean, and comfortable environment for residents while observing infection control practices
• Document care thoroughly and clearly in the patient record to support continuity of care
• Collaborate with colleagues and families to deliver person-centered care that respects dignity and preferences

Qualifications

• Active LNA or T-LNA certification (VT or NH authorization preferred for this assignment)
• Current CPR/BLS certification or willingness to obtain upon hire
• Previous experience in long-term care or similar settings is a plus
• Ability to pass facility-required health screenings and background checks
• Strong communication skills, dependable attendance, and a team-oriented attitude

Education

• High school diploma or equivalent
• Completion of an approved LNA training program

Knowledge & Skills

• Skilled in safe resident handling, transfers, and use of assistive devices
• Knowledgeable about infection control and standard precautions
• Comfortable taking and reporting vital signs and basic observations
• Able to document care clearly and follow written care plans
• Compassionate bedside manner and good interpersonal skills with residents and families
