Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments safely and according to facility policy
• Monitor vital signs and report changes promptly to the care team
• Document clinical care and maintain accurate patient records
• Assist patients with activities of daily living while preserving dignity
• Communicate clearly with RNs, physicians, and family members to coordinate care
• Adhere to infection control and safety procedures to protect patients and staff

Qualifications

• Current, active LPN license in Pennsylvania (or compact state eligibility where applicable)
• Graduation from an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification required
• Ability to pass required background check and drug screening
• Strong communication skills and a collaborative mindset
• Previous experience in long-term care or acute care settings preferred

Education

• Completion of an accredited LPN/LVN program (diploma or certificate)

Knowledge & Skills

• Medication administration and accurate charting
• Proficient in taking and interpreting vital signs
• Familiarity with electronic health records and documentation best practices
• Patient-centered communication and bedside manner
• Infection prevention and control practices
• Time management and prioritization in a fast-paced environment
• Team collaboration with interdisciplinary care teams
