Roles & Responsibilities

• Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
• Administer medications and treatments accurately and document appropriately
• Monitor resident conditions and communicate changes promptly to the care team
• Assist with ADLs (activities of daily living) while promoting resident dignity
• Collaborate with RNs, CNAs, and interdisciplinary team members to deliver coordinated care
• Maintain clear, timely charting in the resident record and follow facility policies
• Support a safe, clean environment through infection control and safety practices

Qualifications

• Active LPN license (PA license expected for this location)
• Current BLS/CPR certification or ability to obtain upon hire
• Comfortable with medication administration, wound care basics, and routine nursing procedures
• Ability to pass background check and drug screening as required by facility
• Strong communication, teamwork, and time-management skills

Education

• Practical Nursing diploma or equivalent (LPN program completion required)

Knowledge & Skills

• Skilled in patient assessment and basic clinical documentation
• Medication administration and adherence to five-rights principles
• Familiarity with infection control practices and safety protocols
• Ability to work in an electronic medical record (EMR) system or adapt quickly
• Good interpersonal skills for collaborating with residents, families, and staff
